Here and Now Records: Archive or Living History

Federal law will require digital patient records in just a few years whether we like it or not. The dental record is often just archived words and papers. Records are living documents telling about the past, identifying future opportunities and assisting in making present recommendations. Record keeping is part of every patient contact and appointment.  It is truly an aspect of care, yet it is often glossed over, done as an afterthought or performed so routinely that there is no meaning.
